PATH provides both virtual and in-home care focused on expert pain and symptom management, while allowing patients to continue their ongoing treatments if they choose. Every individual is monitored closely by a palliative care physician, ensuring specialized medical support throughout the journey.

PATH accepts referrals from a wide range of sources - not just doctors. Nurses, social workers, Ontario Health at Home Care Coordinators, and even family members can connect patients to this service, making access simple and responsive.
