Palliative and Transitional Health at Home (PATH) is a compassionate care service dedicated to supporting people living with life-limiting ailments in the comfort of their own homes. PATH provides both virtual and in-home care focused on expert pain and symptom management, while allowing patients to continue their ongoing treatments if they choose. Every individual is monitored closely by a palliative care physician, ensuring specialized medical support throughout the journey.

PATH accepts referrals from a wide range of sources - not just doctors. Nurses, social workers, Ontario Health at Home Care Coordinators, and even family members can connect patients to this service, making access simple and responsive.

Our goal is to walk alongside patients and families, offering dignity, relief, and continuity of care at every stage of the journey.
